The California Department of Health has issued an All Facilities Letter recalling Environmental Test Systems' SteriChek "Sensitive" Total Chloramines and Residual Chlorine Reagent Strips. The strips may provide inaccurate readings of dialysis fluid chlorine levels, leading to hemolytic anemia and related complications. The recall comes after an adverse event report in Florida, where 31 of 32 patients experienced hemolytic anemia due to inaccurate chlorine readings, with one patient suffering a myocardial infarction. Healthcare facilities, including nursing homes, must immediately evaluate their stock of the affected products, remove them from their facilities, and ensure they are no longer available for patient use. The affected products were shipped on or after August 24, 2006. Facilities should contact their supplier for a list of affected lot numbers.
