Nursing homes/healthcare facilities must coordinate assessments with the Pre-Admission Screening and Resident Review (PASARR) program to avoid duplicative testing. This includes incorporating PASARR Level II recommendations into a resident's assessments, care planning, and transitions of care. Facilities must also refer all Level II residents and those with newly evident or possible serious mental disorder, intellectual disability, or related conditions for Level II review upon a significant change in status assessment. The PASARR process requires that all applicants to Medicaid-certified nursing facilities be screened for possible serious mental disorders or intellectual disabilities and related conditions. If a resident is identified with newly evident or possible serious MD, ID, or a related condition, the facility must refer the resident to the appropriate state-designated authority for review. The state must provide or arrange for the provision of specialized services to all NF residents with MD or ID, and the facility must arrange for off-site visits for services if appropriate. Nursing homes/healthcare facilities must be aware of these regulations and take the appropriate action when necessary.
