Nursing homes and other healthcare facilities must prepare a discharge summary for anticipated discharges that includes a recapitulation of the resident's stay, final summary of the resident's status, reconciled pre and post-discharge medications, and post-discharge plan of care. The discharge summary must be provided to the receiving provider at the time of discharge in either hard copy or electronic format. The discharge summary must include an accurate and current description of the clinical status of the resident and sufficiently detailed, individualized care instructions, to ensure that care is coordinated and the resident transitions safely from one setting to another. The facility must also reconcile the resident's pre-discharge medications with their post-discharge medications and provide written discharge instructions to the resident and, if applicable, the resident representative. Nursing homes and other healthcare facilities must adhere to these requirements to ensure the safe and effective transition of care for residents.
