Updated guidance on COVID-19 vaccinations, masking, screening, testing, isolation, visitation, and infection control requirements - supersedes PIN-23-13-ASC. Aligns the guidance with that of other respiratory viruses and focuses on clinical symptoms to determine when to end isolation - instead of five days.
Masking
Follow CDPH 1/9/24 COVID-19 Isolation Guidance:
After a positive test, mask indoors when around others during the full 10 days following symptom onset date (or positive test date if no symptoms). May remove sooner after two sequential negative antigen tests at least one day apart.
If test results are positive, continue wearing a mask and wait at least one day before taking another test up to day 10.
Optimize mask fit and filtration, ideally through use of a respirator (N95, KN95, KF94) or surgical mask if a respirator is not available.

Screening of Residents and Employees
Requirements to observe for resident changes in condition and symptoms of COVID-19 remain unchanged. Continue passive screening of employees, visitors, volunteers and other individuals who enter the facility.
Testing and Isolation of Residents and Employees
The CDPH 1/9/24 updated testing and isolation guidance applies to all residents.
Residents who are close contacts and do not have symptoms, regardless of vaccination status, do not need to be restricted to their room or cared for by employees using the PPE required for the care of a resident with COVID-19.
Residents with confirmed COVID-19 must continue to be promptly isolated. The facility must have a current infection control plan that addresses isolation protocols. Employees must wear the appropriate PPE and have signage on proper donning and doffing.
Testing and Isolation Recommendations for Employees
A facility should follow the CDPH 1/9/24 Isolation guidance and the Cal/OSHA regulations for testing close contacts, outbreak/response testing, work exclusions and return-to-work criteria. There are optional additional protective procedures that may be implemented.
Continue to have a contingency plan for staffing shortages, including consulting the DSS Regional Office and local health department when the facility has a staffing shortage.
Visitation
Continue to follow the bet practices for visitation related to COVID-19 screening, masking and PPE.
Visitors should review the CDPH Masking guidance above. For residents in isolation, the facility should provide the visitor with the same type of PPE used by employees. A visitor does not need a fit tested N95 respirator, but should be instructed on how to perform a seal check.
Infection Control Requirements
A facility must continue to follow regulations related to infection control, prevention and mitigation for communicable diseases. This includes the requirement to have an Infection Control Plan that includes an infection control training plan, guidelines related to environmental cleaning and disinfection, use of PPE, and hygiene and cough etiquette.
When an emergency for a communicable disease is proclaimed or declared, a facility must develop an Emergency Infection Control Plan that includes infection control measures that are not already addressed in their Infection Co note Plan - see PIN 23-12-ASC.
