The California Department of Health has issued a new All Facilities Letter (AFL) stating that the oversight responsibility of certain healthcare facilities in Santa Clara County will be transferred from the San Francisco District Office to the San Jose District Office. Effective October 1, 2017, the San Jose District Office will be responsible for overseeing the listed facilities, including Adult Day Health Centers, Ambulatory Surgery Centers, Chronic Dialysis Clinics, Home Health Agencies, Hospices, and more. Healthcare facilities affected by this change must update their records to include the San Jose District Office's information. All complaints and entity reported incidents should be reported to the San Jose District Office following the transfer. Non-compliance with these changes may result in penalties or other consequences. Facilities should contact the San Jose District Office if they have any questions about the content of this AFL.
