The California Department of Health has issued an All Facilities Letter announcing the consolidation of licensing applications and report of change requests to the Centralized Applications Unit (CAU) in Sacramento. The transition began on July 1, 2015, and is expected to be completed by February 1, 2017. As of October 20, 2016, CAU has assumed the application and report of change request workload for several district offices. CAU will review and process various facility-related applications and changes, including initial/renewal applications, change of ownership, change of location, change of mailing address, change of name, adding/deleting services, and change of director of patient care services. District offices will continue to perform any required licensing and certification surveys. Facilities should submit all requests to CAU at the specified address and contact CAU for any questions regarding the application approval process.
