The California Department of Health has issued a new All Facilities Letter (AFL 13-27) announcing a change in the collection address for Licensing and Certification Program Enforcement Penalties. Effective January 1, 2014, healthcare facilities, including nursing homes, must send payments for enforcement penalties to the new address: Department of Public Health, Licensing and Certification Program, Grant & Fiscal Assessment Unit, P.O. Box 997434, Sacramento, CA 95899-7434. This change applies to citations, immediate jeopardy administrative penalties, medical breach administrative penalties, medical breach failure to report administrative penalties, adverse event failure to report administrative penalties, and nursing home per patient day penalties. Facilities should update their records accordingly and direct any questions to Cli Donaldson at (916) 552-8649 or cli.donaldson@cdph.ca.gov.
