The California Department of Health has issued a new All Facilities Letter (AFL) reminding home health agencies of the mandatory criminal record clearance requirements for owners, administrators, and administrator's designees. These individuals must submit a Request for Live Scan Service: BCIA 8016 form and complete the live scan process with a live scan vendor. The completed CDPH 325 Criminal Record Clearance Submission form, along with proof of live scan, should be submitted to the Licensing and Certification Centralized Applications Unit before February 28, 2012. Failure to comply with these requirements may result in a delay or denial of license renewal, with no extensions allowed under the law. The AFL serves as the final notification and reminder of the information previously communicated in AFLs 11-06 and 11-53.
